And suddenly the skeleton was no longer in the closet. Rather, she stood before me, face ashen with the realisation that her secrets had been discovered, nothing between me and her bones but crepe-paper skin and ever-thinning air, no longer satisfying my lungs. Her skin was now translucent, streaked with a road-map of blue veins, coated with fine hair reminiscent of the moleskin skirt she had once so favoured. Emaciated, she stared back at me, too startled to feel the shame of standing naked before her sister.

"Don't tell," she whispered, voice pleading, eyes simultaneously blank and brimming, as though she knew she ought to feel emotional, but had already checked out. "You can't tell. Not if you love me."

I'd describe her bones as being like roots of trees running so close to the surface of the earth that they show through, that only a downpour of autumn rain was needed to bare them completely; I'd also use something to describe the shock, such as saying 'I stood staring after her for some seconds, unable to fully accept what I'd just seen, wanting even to grab the towel away that she'd so hastily tucked round her when we collided by the bathroom door just to be sure this emaciated figure more like a famine victim than a normal healthy teen, actually was my sister'.

In a book? What do you mean? Are you writing a book about an anorexic? I'd use the words "only a ghostly, skeletal shadow of the healthy girl I once knew as my sister".

It depends on the degree of ribs coming out to be considered shocking. Many people who are not anorexic have some ribs popping out. If you see every rib and every other bone coming out then it is to be considered shocking. But, a couple of ribs is not shocking.
