Ooh, tough one.

PB stupidest: Trusts someone she knows is on the other side, despite her instincts (and he ends up betraying the good guys)

Smartest: Cracks the riddle and finds the evil queen's weakness.

II stupidest: He's pretty stupid to begin with, but his worst mistake is abandoning his best friend because of a bunch of lies from someone he doesn't like, trust or respect.

Smartest: Deciding to save her anyway.

I have two main characters :)

Stupidest:

Male character - him assuming that there would be zero consequences of him being an alcoholic, and the assigned Protector of the other MC who is an assumed target of an extremist group. Basically, he had someone else's life in his hands and was drunk all the time

Female character - to put this briefly, she caused the death of another character when she stopped barricading a door and instead decided to run away. That or trying effectively to break into a hospital in what was perhaps the stupidest way possible

Smartest:

Male character - gets the other MC out of her stupid 'hospital' break in attempt. That, or successfully luring the antagonist out into the open, but it didn't end well so it was only smart if you only consider the fact that he managed to draw him out and ignore what happened after

Female character - she successfully rescues this guy ^ from a near-death situation

The stupidest thing was something she did in the past, before the story starts. She was careless with her finances and get herself into a debt she could never pay off, basically ruining her life.

The smartest thing (and this is going to sound incredibly ironic, considering what you just said about your own story) was probably to allow herself to be subjected to an experiment by the people she met.

Smartest: Female MC - Stays to protect her boyfriend, an ends up saving his life

Male MC - Figures out how to stop the criminal organization

Stupidest: Female MC - Stays to protect her boyfriend, and ends up loosing her life.

Male MC - Figures out how to stop the huge criminal organization, that he doesn't know his girlfriend works for, which ends up being the cause of her death.

Yes their smartest thing is also their stupidest.

Stupidest: She sneaks away from home to fight in a revolution - when she's already declared that she's neutral and shouldn't be fighting in it at all... o_o She only goes to try and help out her friend and godfather, who are on opposite sides... o.e -, dresses in boy's clothing and as she's helping her godfather fight, doesn't even bother to deepen her voice... and then once the godfather realizes its her, -then- she deepens her voice.... X)

... yeah, I know I did a bad job of summarizing it... ^^;

Smartest: Realizes that she can't live her life afraid of loving people and getting attached, and tries to open up more and let people in...

Stupidest: Let her friend teach her how to ride a bike on the street when her mom told her specifically not to.

Smartest: Listening to her mom the rest of the time.

I'm only on Chapter 2 in the story.

Stupidest: She tries to swim to impress this guy, but in reality she really doesn't know how to swim, and she ended up going to the hospital.

Smartest: She gives the idea of changing their looks, so that the enemy won't recognize them, since their enemy is a shape-shifter, so that evens out the competition.

Smartest: She calls the Sand Naga Queen's bluff when she tries to bring Alyzah back to Arteas when she falls sick.

Stupidest: She hangs onto an egg-thing that suddenly starts playing when she's near it, only to learn later it could have sucked her soul out in her sleep.

Stupidest: It was probably the decision to return to Earth, against her orders. She lost all respect after that, despite the fact she swiftly returned.

One main character in my trilogy of books does something really stupid, and another main character in that same trilogy did something smart:

Stupidest: not realizing when one of their family members was really sick, and the main character who didn't realize it had had the same thing. He/She was also not there at the time the family member was sick due to work. (Luckily, he/she then returned home.)

Smartest: when the character was trying to train for something, the enemy tried to "help" her out by using a loved one, also a main character, as practice (they'd kidnapped him/her). The main character who refused then rescued her loved one without hurting them while tricking and defeating the enemy.

My character doesn't do stupid or smart things; she's just caught up in the action around her and doesn't really ever have the opportunity to make choices.

Wow, you must have an...interesting story. I don't think anyone could possibly call that cliche.

in my stories my character usually says something she/he shouldn't say or is talking to a friend about some one to a friend and another person hears then it spreads around.

in action books: they usually do something stupid like drop a weapon or loose there only hope at survival, or break a important alliance

smartest:

they do a lot of clever things so i don't really know...
